P.F. Percy, chief surgeon of the English Army... and currently of that of Mainz, attests that citizen Jean Georges Weiss, aged 37, native of Ingersheim, Bas-Rhin department, was employed as a third-class surgeon at the ambulant hospitals of the Rhine Army, which is now part of that of Mainz, from the 6th of Pluviôse Year 2 until the 1st of Germinal Year 4, at which time the Union Hospital in Strasbourg, to which he was attached, was closed. He was discharged along with many surgeons of all ranks, who had become surplus like him.
In witness whereof, this document is issued to him to serve and be valid as a matter of justice and reason.
