The piece features the central male figure, identifiable as Elvis Presley, "The King" himself, recognizable by his face and famous banana haircut. He is dressed in an ornate Japanese-style robe, fusing his iconic gold-studded style with intricate embroideries of peacocks and gold threads, positioning him as a shōgun or a high-ranking noble. He holds a Japanese fan bearing the rising sun motif in red. He is surrounded by four Japanese women in geisha or traditional courtesan attire (colorful kimonos, elaborate hairstyles). Two kneel and gaze at him with admiration, while another adjusts his robe. The background is an opulent interior of a Japanese audience hall (zashiki) with patterned ceiling panels, a gold backdrop, shōji screens, tatami mats, and hanging scrolls of Japanese calligraphy and wall paintings.

Note the authentic detail of the signature "Medsan" calligraphed in Japanese characters (katakana) in a traditional cartouche located at the top right of the composition (メドサン), as well as the artist’s manual signature and numbering on the back.
