This piece features a distinctive, ornate quatrefoil-inspired shape with a finely detailed relief design on both sides:

- Obverse (Front): Depicts a detailed high-relief image of Our Lady of Help (N. Senhora da Ajuda, Padroeira de Espinho) holding the Child Jesus, surrounded by an inscription framing the border.

Depiction: A detailed relief representation of Our Lady of Perpetual Help (Nossa Senhora do Perpétuo Socorro) in the traditional Byzantine iconographic style, holding the Infant Jesus, accompanied by sacred monograms and archangels.
Inscription: Around the oval border reads: N. A S. A DO PERPETUO SOCORRO ROGAI POR NOS (Our Lady of Perpetual Help, Pray For Us).

- Reverse
Depiction: Features Saint Alphonsus Liguori (Santo Afonso de Ligório), the founder of the Redemptorist Congregation, who played a central role in promoting devotion to Our Lady of Perpetual Help. He is depicted holding a radiating monstrance and a rosary.
